As part of our new virtual primary care platform, this role will work under the direct supervision of the Care Team Supervisor. The Certified Medical Assistant is responsible for providing patient care and assistance to patients, providers and pharmacies.
Assist patients via telephone as part of primary care team including call backs and providing normal test results per provider instructions
Assist in patient care plans and chronic disease management through the collection of health information, vitals and other health history information that needs to be shared with the provider and other care team members. May give the provider’s advice to the patient, however the CMA does not perform telephone triage
Provides health care support services for all patients (adult and paediatrics) as established by clinic protocols and community practice standards
Assist in patient care coordination with patient referral tracking, goals and education
Handles calls and correspondence regarding pharmacy issues from physicians, pharmacies and patients.
Identifies and/or responds to inquiries, prescription issues, changes and refills
Document calls in case management system using established protocols to ensure resolutions provided are presented in a clear and accurate manner
Actively manages quality initiatives in collaboration with provider and clinical care staff
Maintains quality assurance logs
Creates and supports an environment which fosters teamwork, cooperation, respect and diversity.
Establishes and maintains positive communication and professional demeanor with co-workers & patients at all times
Performs other related duties and projects as assigned. Assist with training of new hires. Attends required training as requested. Develop and maintain desktop procedures
2+ years experience in a healthcare, prefer a primary or specialty program working with chronic disease management
High School diploma or the equivalent
Graduate of accredited medical assistant program
