Key Responsibilities

  • Time Management

  • Understanding the Aged Care Standards

  • Liaising with stakeholders and internal networks 

What you bring 

  • Evidence of a minimum of 1 years experience in Case Management

  • Relevant tertiary qualification in the community sector

  • Demonstrated experience in the aged/community/disability sector particularly working with clients with complex needs

  • Full Drivers Licence and own reliable registered and insured Vehicle 

  • Smart Phone

  • Knowledge of My Aged Care portal/assessment process 

  • Willing to work in a team environment 

  • A desire to join a team committed to the care of others in a mission based organisation

  • NDIS clearance (or willingness to obtain one)

  • Police clearance (within the last 6 months Or willingness to obtain one)
