Overseeing a client caseload under CHSP/HCP approved funding is the responsibility of the position. budgetary control, managing care needs to preserve clients' independence, and welcoming new clients.
Time Management
Understanding the Aged Care Standards
Liaising with stakeholders and internal networks
Evidence of a minimum of 1 years experience in Case Management
Relevant tertiary qualification in the community sector
Demonstrated experience in the aged/community/disability sector particularly working with clients with complex needs
Full Drivers Licence and own reliable registered and insured Vehicle
Smart Phone
Knowledge of My Aged Care portal/assessment process
Willing to work in a team environment
A desire to join a team committed to the care of others in a mission based organisation
NDIS clearance (or willingness to obtain one)
Police clearance (within the last 6 months Or willingness to obtain one)
